Applicants from the State of Alaska, Washington, Oregon, Nevada and Northern California need to apply to the Consulate General of the People's Republic of China in San Francisco.

As part of the application documents, a Foreigner Physical Examination Form should be included in the application package and it is available here. Please refer to the instructions for detailed requirement for it.

Online application and the corresponding application documents should be submitted to us no later than February 29, 2020.

Please note, application documents needs to be bound on top left corner (in duplicate), and no application documents will be returned. Documents that are incomplete, untrue or fail to meet the requirements will not be accepted.
