As part of the application documents, a Foreigner Physical Examination Form should be included in the application package and it is available here. Please refer to the instructions for detailed requirement for it.

Online application and the corresponding application documents should be submitted to us no later than March 15, 2021.

Due to the pandemic, all documents should be submitted online. The original or duplicate copy might be required upon request.
